Case Initiation Request Forms for University Employees
Choose the type of case request below. If necessary, you may save the form and return to it later.
Nonimmigrant Visa Case Initiation Request Form (H-1B, E-3, O-1, TN)
For the Nonimmigrant Visa Case Request, you will need the following:
- a completed Visa Deemed Export Questionnaire
- Job description
- Employee’s current status and expiration
- Employee’s contact information
- COA/POET for charges
- Name and email of department contact, supervisor and chair
Permanent Residence Case Initiation Request Form
For the Permanent Resident Case Request, you will need the following:
- Job description
- Employee’s current status and expiration
- Employee’s contact information
- Information on similarly employed employees (same department, job title, and similar education and qualifications)
- Name and email of department contact, supervisor and chair
Fees
The department is responsible for all fees related to the status requested for the employee being sponsored.
Dependent costs (spouse and children) are paid for by the sponsored employee unless otherwise specified by the department. Premium processing fees may also be paid by the employee in some circumstances.
