Membership Cancellation

All memberships have been put on hold due to the Covid-19 pandemic. Members wishing to cancel their membership must please fill out the Membership Cancellation Request form below.

  • Once the Membership Cancellation Request form has been submitted, the member will receive an email confirmation of their canceled membership.
  • Cancellation requests must be submitted for each individual person of a family membership.
    For example: If you have 3 persons in your family with a current membership and wish to cancel all memberships, 3 cancellation requests must be submitted; 1 for each person.
