Vanderbilt employees may request reasonable accommodations through EOA. To request accommodations, faculty and staff should complete the reasonable accommodation request form and have their medical provider complete a Medical Request Form. Receipt of incomplete information will cause a delay in assessing the employee’s request. EOA will contact the employee once the medical documentation has been received. Medical information or other details included in an accommodation request will only be shared on a strict need-to-know basis with the necessary personnel to consider the implementation of the accommodation. Factors to determine whether an accommodation is reasonable and necessary include the employee’s specific request, the timing of the request, the supervisor’s input, the essential job functions, and the content of the supporting medical documentation. All requests are reviewed and assessed on an individual basis. If a change in job, medical condition, or another accommodation is desired, it may be necessary to submit new documentation for review.
