Founder’s Medal Submission Form

Please share information on your 2024 Founder’s Medalist using the form below. The information you share will be used to develop text for the printed/digital Commencement program, the provost’s remarks and any additional communications and recognition of the medalist.

Deadline: Submissions are due no later than Friday April 19, 2024.

Used for: This will be the information used to recognize the students and in the program during Commencement.


  • Please include the phonetic spelling of your student’s name.
  • If there are particular points you want to be highlighted in the program listing or provost’s remarks, please note that on the form.
  • Before the text is placed in the program or script, we will ask you to proof the final copy.
  • Please only submit information requested on the form. Do not submit any additional documents.

If a final recipient has yet to be chosen by the April 19 deadline, please submit a separate form for each of the final two candidates. As soon as the medalist is confirmed, please reach out to This information will be kept in strict confidence.
