Checklist of F2P2 Requirements

Upon completion of the program, the following documents should be placed in your F2P2 notebook:

Professional Development Track

1. Form 1: Documentation for Workshops - Six workshop hours (one form per workshop)

2. Questions and responses for one interview with a Vanderbilt faculty member on faculty roles and responsibilities

3. Form 2: Event Attendance Form - Faculty meeting

4. Name and date of conference or Graduate Student Research Day attended and abstract of presentation

5. Interview documents:

   1. Teaching philosophy statement
   2. List of items in teaching portfolio
   3. Sample cover letter
   4. CV

6. Questions and answers for partner institution faculty interviews (same as those in the Teaching & Learning track)

7. Partner institution visit essay (one essay covering both visits)

Teaching & Learning Track

1. Form 1: Documentation for Workshops - Six workshop hours (one form per workshop)

2. Form 2: Event Attendance Form - Videotape consultation

3. Form 2: Event Attendance Form - Faculty-supervised teaching and reflection

4. Questions and responses for one interview with a Vanderbilt faculty member on teaching

5. Questions and responses for one discussion with a fellow F2P2 participant on teaching

6. Mentoring essay

7. Teaching documents (for five hours of teaching)

   1. Lesson plans
   2. Classroom or student assessment
   3. Reflection

8. Workshop documents

   1. Announcement flyer
   2. Lesson plan and copy of handouts
   3. List of attendees
   4. Summary of assessment

9. Questions and answers for partner institution faculty interviews (same as those in the Professional Development track)

World of the University Track

1. Form 1: Documentation for Workshops - Six workshop hours (one form per workshop)

3. Summary of community service project (one page)

4. Form 2: Event Attendance Form - Community service project

5. Questions and answers for partner institution administrator interviews
