NEW: Effective Friday, September 20, 2013, we will initiate a new process for review of expedited amendments, adverse events, and non-compliances (NCP). Electronic signature stamps from the Committee Chair or designee indicating approval for these submissions will now be placed at the bottom of amendment, AE, and NCP forms. Please ensure that your amendment, AE,…
Effective Friday, August 16, 2013, we will be stamping the approval date on all materials (e.g., ads, applications, supplemental information, etc.) that go to Committee for review, once approved. Protocols and Investigator’s Brochures will not be stamped since these documents are always the sponsor’s confidential information but will be referenced in the final approval letter.
