This hold prevents you from registering for classes until you have provided your address. You can provide your local address in the "Address Change" module in YES. Student Health Hold	Prohibits all enrollment activity	You are not compliant with Vanderbilt University and State of Tennessee requirements for immunizations [proof of 2 Measles, Mumps, Rubella (MMR) vaccination for positive titer results and/or proof of 2 Vaccinations for Varicella, positive titer results or disease documentation of Chicken Pox ]. The Meningococcal ACYW vaccination (administered after age 16) is also required for on-campus residents. You are also required to provide a tuberculosis risk assessment form and have TB testing completed, depending on the results of your survey. Note, if you are an incoming graduate/professional student at Vanderbilt and did your undergraduate work here as well, you may still need to submit your proof of immunizations and complete the TB risk assessment survey, as the requirements in the state of Tennessee may have changed since you were first a student at the University.
