Confidentiality and Privacy

Each visit to VUSHC is confidential. This means that nobody can have access to your file or discuss your case by phone without your written permission. There are pre-printed forms available on-line for transfer of medical records to and from VUSHC. VUSHC requires that you make an appointment with a provider to review the portions of the medical record that you wish transferred to another clinic. There is a copying charge for records released directly to the student, but no charge is incurred for direct transfer of records from VUSHC to another health care provider.
