STUDENT ACCOUNT FORMS

 

STUDENT ACCOUNT AGREEMENT: All incoming undergraduate and graduate/professional students must complete a Student Account Agreement.


FEDERAL STUDENT FINANCIAL AID CASH MANAGEMENT AUTHORIZATION: Title IV funds (Stafford Loans, Perkins Loans, PLUS (Parent) Loans, Pell Grants and Supplemental Grants) can be applied only to tuition, fees and contracted room and board, unless you have authorized applying these funds to your discretionary charges.


UNDERGRADUATE REFUND REQUEST FORM: Students who have a credit balance on their student account must complete and submit a written request for those funds to be refunded to them.


STUDENT HEALTH INSURANCE FORMS: Students who have previously waived University-sponsored student health insurance can purchase it at any time by submitting an acceptance form.
