Privacy Policy Regarding Vanderbilt’s Health Risk Assessment Data

We understand that information about you and your health is personal and private. We are committed to protecting information about you.

Healthy Lifestyle Feedback Directly to You.

The primary purpose of the Health Risk Assessment and the Go for the Gold Incentive Program is to help you identify your individual health risks and to learn about how you can maintain or improve your health.

List of Eligible Employees

HEALTH Plus will use the list of participants who have completed their Health Risk Assessment to give eligible participants Vanderbilt Wellness Benefits such as access to the HEALTH Plus exercise facility and the Go for the Gold Wellness credit.

HEALTH Plus Will Provide Those with Health Risks with Information on Available Resources and Other Benefits.

If you have health risks, this information will be used by HEALTH Plus to provide you with additional resources unless you indicate that you do not wish to be contacted on question 39 of the Health Risk Assessment.
If you indicate that you do not wish to be contacted, no one at HEALTH Plus will contact you. If you decide later that you would like to be contacted, you may notify HEALTH Plus.

Assessing the Cost-Effectiveness of the Go for the Gold Program and other Health Promotion Programs.

HEALTH Plus uses anonymous group data to plan and prioritize programs and services to meet the health needs of the Vanderbilt employee population, therefore helping to develop a culture of wellness. The analysis will be done using aggregate data (your data combined with those of other participants which does not personally identify you).

HEALTH Plus Partners with Wellsource to Provide the Online Health Risk Assessment

Wellsource and HEALTH Plus are committed to complying with the Standards for Privacy of Individually Identifiable Health Information under the Health Insurance Portability and Accountability Act (HIPAA) of 1996. HEALTH Plus has partnered with Wellsource to provide the Health Risk Assessment since 1991.

Prohibited Uses

Confidential individual Health Risk Assessment information will not be provided to any insurance companies, life insurance companies, automobile insurance companies, any outside companies or any Vanderbilt departments, managers, or supervisors.
