Alcohol Registration Form
This form is provided to clarify what is required for an alcohol-related event; plan accordingly.
This form does not take the place of registering your event with the Office of Schedules and Reservations. You must go to 208 Sarratt or call 2-2448, to reserve a room or other requirements. After you arrange the logistics, you will attend the Special Events Registration Committee, which meets every Wednesday from 2-3:30 pm (except during the summer months and winter break.) Any questions should be directed to the Office of Schedules and Reservations.
Print out this form, fill it in, and bring it to the Special Events Registration meeting after you have communicated with the Office of Schedules and Reservations. The form is a PDF document; you will need the free Acrobat Reader to access the file.
